After 7 years of clinical use, percutaneous transluminal coronary angio plasty (PTCA) has now found worldwide acceptance, and its basic techniques are well standardized. A growing number of invasive cardi ologists are acquainting themselves with the procedure in order to be in a position to offer new therapeutic options to their patients. However, the transition from the diagnostic to the therapeutic intervention is not always easily accomplished. This volume contains the proceedings of the Fourth International Symposium on Coronary Heart Disease, held in Frankfurt/Federal Republic of Germany. In 1970, when the fIrst symposium was held, the whole range of diagnostic and therapeutic measures was covered, but since then interest in coronary heart disease has increased so considerably that the fourth symposium was restricted to only two therapeutic aspects, angioplasty and intracoronary thrombolysis. It can be noted with pleasure that European cardiologists, namely Andreas Griintzig and Peter Rentrop, have made essential contributions to the applicability of these modes of treatment. When angioplasty was fIrst discussed at the Third International Symposium (Coronary Heart Disease, Stuttgart, 1978) a great deal of astonishment was expressed. Today the procedure has gained an important place in the treatment of angina pecto ris. The proportion of candidates for coronary surgery in whom angioplasty can be attempted instead of operation is around 10%, in certain subgroups even higher. The most remarkable fmding of our 5-year experience with angioplasty are the extre mely good long-term results. Recurrences happen in about 15%-20% of cases within the fIrst 3 months; after this period of time, recurrences are very unusual and mostly due to progression of the disease at other sites. Angioplasty has thus also contributed to our understanding of the dynamics of the atherosclerotic process.

Since the introduction of coronary angioplasty in 1977, this procedure has gained a steadily increasing position in the treatment of coronary artery obstmction. From the available evidence it can be estimated, that this thera peutic tool will get even more additional momentum of many ten-thousands of patients to be treated in the next few years, due to a growing fraction of patients who are candidates for this intervention. Information about the indications, benefits and risks of coronary angioplasty is accumulating rapidly in addition to publications about refinements of the technique itself. Recently, a number of investigators have realized that coronary angioplasty is not only a therapeutic tool, but can, during the procedure, be used as a source of diagnostic information. When the catheter is placed in a coronary artery obstruction, inflation of the balloon produces transient myocardial ischemia. Before, during, and after this period of severe ischemia, studies of the perfor mance of the myocardium at risk can be carried out. The fact that therapeutic coronary angioplasty is carried out in a cardiac catheterization laboratory which is by definition optimally equipped for the measurement of hemodynamic parameters, has probably also contributed to the effectuation of these investigations. The combination of hemodynamic and biochemical parameters with morphological information from the coronary angiogram can be utilized for the quantification of myocardial involvement and the success of coronary dilatation with angioplasty. Studies of interactions with pharmacological substances are also feasible and informative.
